When you first come to Lifeplan, we take our time to get to know you, so we can ensure you receive the right supports, to help you live your life, your way. While our approach is completely tailored, below is an example of what you can expect when you first meet with Lifeplan, as we guide you through our approach:

Meet a Coordinator

When you first meet with Lifeplan, you will be designated a personal Coordinator, who will be with you throughout the process. Your Coordinator is your point of contact for all communications between you, Lifeplan and the NDIS. Our Lifeplan Coordinators have years of experience working with the NDIS to achieve the best outcomes for our customers. You can be confident your coordinator has your goals as their number one priority, as they help you develop your life plan.

Develop a Vision

Your Vision is your dream living scenario. We want to help you create a vision for your future, that sees you living your best life. Lifeplan pride ourselves on being positive in everything we do and helping our customers and their families create and achieve their Vision both now, and for the future.

You vision should include some clear goal setting, as this is required when you have your NDIS planning meeting. It’s important you clearly understand and communicate your goals to your NDIS planner, to ensure they put you on the right plan. Lifeplan are happy to help you put together your goals for your NDIS meeting, and often attend the NDIS planning meetings with your customers, to ensure everything is communicated correctly.

Explore the Options

There are no cookie cutter service options to choose from at Lifeplan. After taking the time to listen to your vision, we will help explore your options, to find the best fit for you. We are driven by your vision to supply the best one-to-one supports to meet your needs. We work closely with you, your family and support network, the Department of Communities and the NDIS, to ensure we explore all your options, to find the best outcome. Develop a Plan

Using our expert knowledge of the NDIS and the industry options available to you in Perth, we will help you develop a support plan to suit your Vision and goals. This plan will be developed in conjunction with your NDIS funding and other service providers, to meet your requirements.

Make it Happen

Once you have your plan, it’s time to make it a reality. Our experienced staff are experts in implementing your NDIS plan and ensuring it matches your original vision and goals. There is no ‘set and forget’ service at Lifeplan, our team will be with you every step of the way. We will communicate what is happening and make sure you and your family are kept informed at every stage of the implementation process.

Review the Plan
